Project Lifeline assists our families in navigating services for children with special needs through intensive case management. After an initial assessment is completed, the Social Worker will work collaboratively with your family to create an individualized plan for programs and services. Individualized support is provided to each family by our Social Worker who can assist you in connecting to resources within the community that support your child and your family needs.
All Project Lifeline services are provided free of cost to our families.
